Yes! If you are entering into one of our cardiac rehabilitation programs, a referral from your doctor is needed before we get started. As with most cardiac rehab programs, getting the green light from your doctor, as well as your cardiac history, is needed so we can keep you safe, and offer you the best standard of care. Most doctors are more than happy to refer, as they know their patients will receive the care they need to get better. We can take care of this step for you!

What we need is your doctors ok to participate in a regular exercise program. Our referral form is an easy way for your doctors to give us that ok, as well as a little health history about you so that we can take better care of you.

You can download the Physician Referral Form directly from our site and take it in to your doctor. They can then fax it to us. Or we can take care of this step for you.
